Scheduling an Appointment
Scheduling an appointment is easy! There are different processes for new patients, returning patients, and current patients. For any questions, or to make an appointment call 734-224-7073
Patient Type
New Patient
For new patients, we will need a referral for physical therapy from your doctor. You can bring it with you to your first appointment, or they can fax it to us. You will need to call to schedule an appointment. We will take your insurance information during this call. This is in order to get authorization for your visit. Please bring your insurance card, referral, and ID to your first appointment.
Returning Patients
Since you are a returning patient, we will most likely already have your information on file. We will need a new referral from your doctor and any current insurance information. Please call to schedule an appointment.
Current Patients
As a current patient, all you need to do is call to schedule an appointment. We can also schedule one while you are in the office. Please let us know if anything comes up and you need to change or cancel an appointment. Thank you for your business!
Insurance and Payment
We accept many different forms of insurance, but we do not take Paramount HMO or Priority Health. Depending on your insurance and coverage plans, you may have a copay for your visits. Please contact us if you have any questions or doubts about getting approval from your insurance company. Self-pay is also an option. It is currently $85 per visit for self-pay. There is an additional $95 due for an initial evaluation for all new patients.
